1. What is concierge mobile physical therapy?
Concierge physical therapy with A.D.M. is a specialized, premium model of care where clients invest out-of-pocket or via membership fee for direct, highly personalized access to Anthony, PT.
Mobile physical therapy with A.D.M. is where Anthony brings the clinic to directly to your home or another convenient location to deliver one-on-one physical therapy.
2. Who can benefit from concierge PT?
Concierge PT can be a good option for adults who want highly individualized care, have difficulty attending a traditional clinic, or would prefer to have the clinic brought to their location. This may include people recovering from surgery, managing back, neck, shoulder, hip, or knee pain, working on balance or fall prevention, returning to exercise or sports, improving mobility, or seeking support with healthy aging.
3. Do you take insurance?
Yes. A.D.M. Doctor of Physical Therapy, LLC accepts traditional Medicare Part B (red, white, blue card), Medicare Part B covers 80%, supplement covers 20%. If you have Medicare advantage plans and other insurances, we are out of network and we provide a “superbill” for you to submit to the insurance company for reimbursement. To determine what you will receive after you submit the superbill, we recommend calling the number on the back of the insurance card and asking them how much they pay for out of network physical therapy appointments. Each insurance plan is different and for this reason, the call you make will be more accurate.
4. Do I need a PT prescription/referral?
For Medicare Part B Beneficiaries, yes, it is required. For all others, it is helpful but if you do not have one, the doctor can call or fax. Call to speak with Anthony directly for a verbal order: 239-944-6318; Fax 239-244-9067.
5. Where do appointments take place?
Appointments can take place in your home, condominium or apartment building gym, workplace, private gym, assisted-living community, or another appropriate location. The setting is selected based on your goals, safety, privacy, and available space. Depending on what you are being seen for, we can discuss how much space will be appropriate. Not all sessions are the same.
6. Can I receive concierge PT while receiving home health or outpatient physical therapy services?
Possibly, but this must be reviewed carefully before care begins. Medicare rules can restrict separate outpatient therapy billing while a patient is actively enrolled in a home-health plan of care, because home-health services are paid under a bundled arrangement. We will coordinate with you and, when appropriate, your physician or current care team to avoid duplicated services and billing conflicts.
7. What conditions do you treat?
Common reasons people seek concierge PT include:
- Back, neck, shoulder, hip, knee, ankle, and foot pain

- Difficulty walking, climbing stairs, getting up from a chair, or moving safely at home

Your physical therapist will determine whether PT is appropriate for your concerns and refer you to another provider when needed.
8. What happens at the first appointment?
Your initial visit includes a detailed conversation about your medical history, symptoms, goals, daily activities, and previous treatment. Your physical therapist will then assess movement, strength, flexibility, balance, walking, functional tasks, and any other areas relevant to your goals.
You will leave with a personalized plan that may include hands-on treatment, exercise, mobility training, pain-management strategies, education, and a home exercise prescription.
9. How long do the appointments last?
The evaluation can last 1 hour to 1.5 hours, depending on your needs. Treatment sessions can last anywhere from 40-90 minutes depending on your condition, goals, plan of care, matched to your preferences and aligned schedules.
10. Will physical therapy hurt?
Some exercises or movements may create temporary muscular fatigue, stretching discomfort, or mild soreness as you improve mobility and strength. Therapy should not involve ignoring significant or worsening pain. Tell your therapist immediately about symptoms such as sharp pain, dizziness, shortness of breath, chest pain, numbness, new weakness, or other concerning changes. Your plan is adjusted based on your response, comfort, and progress.
11. How often will I need visits?
The right frequency depends on your diagnosis, needs, goals, and progress. Some people benefit from more frequent early visits after an injury or surgery, while others need periodic check-ins and a structured independent program. Your plan will be reassessed regularly, so care remains clinically appropriate and aligned with your goals.
12. Is concierge PT only for older adults?
No. Concierge PT serves adults across many activity levels and stages of life. It can be helpful for busy professionals, active adults, athletes, post-operative patients, people managing chronic pain, and older adults who want to maintain strength, mobility, balance, and independence.
13. How do I know if concierge PT is right for me?
Physical therapy may be appropriate if pain, stiffness, weakness, balance issues, or mobility limitations are affecting your work, exercise, sleep, independence, or quality of life. An evaluation is the best way to determine whether PT can help and what a realistic plan would look like. Concierge PT is not a substitute for emergency care. Call 911 or seek urgent medical evaluation for chest pain, sudden shortness of breath, signs of stroke, severe injury, sudden severe headache, loss of consciousness, or other urgent symptoms.

15. Do you have a cancellation policy?
A.D.M. Doctor of Physical Therapy, LLC requests notice for cancellation as soon as you are able to do so.
We understand life “happens” and there are variables beyond our control. Anthony honours that without a fee or surcharge. However, if there is a trend or pattern for cancellation, let us reconnect to problem-solve solutions or if concierge physical therapy may not be the right fit at that time.
