Section 435.04, Florida Statutes mandates Level 2 security background investigations be conducted on employees, defined as individuals required by law to be fingerprinted pursuant to Chapter 435, Florida Statutes.

Authority: The FBI’s acquisition, preservation, and exchange of fingerprints and associated information is generally authorized under 28 U.S.C. 534. Depending on the nature of your application, supplemental authorities include Federal statutes, State statutes pursuant to Pub. L. 92-544, Presidential Executive Orders, and federal regulations. Providing your fingerprints and associated information is voluntary; however, failure to do so may affect completion or approval of your application.

The logo for A.D.M. Doctor of Physical Therapy, LLC is currently patent pending. The servicemark application was filed with the U.S. Patent and Trademark Office (USPTO) in November 2024.

A.D.M.’s novel logo design is legally protected under intellectual property law. The brand name, logo, and other identifiers are not to be shared, manipulated, reproduced, used, or altered in any form by any means, or otherwise without the express permission of A.D.M.

Unauthorized use of the logo is a violation of intellectual property and punishable by law.

A.D.M. utilizes HIPAA compliant platforms including Google Workspace, Microsoft 365, Square and fax company. A.D.M. also has a Business Associate Agreement with Constant Contact.
