Providers Refer a Patient

When your patient needs post-surgical rehabilitation, orthopedic recovery, or in-home physical therapy following a hospital stay, Anthony Daffner-Milos, Doctor of Physical Therapy, provides concierge, one-on-one care delivered directly to their home in Naples, Marco Island, Port Royal, Old Naples, and surrounding Southwest Florida communities.

Anthony communicates directly with referring providers throughout the course of care, sending evaluation summaries and progress notes so you stay fully informed on your patient’s status and recovery.

How to Send a Referral
Referring your patient is straightforward. Please choose the method that works best for your office

Option 1: Fax
Send the PT prescription and most recent office visit note to:

Following the referral, Anthony will conduct an in-home evaluation and fax the evaluation report and ongoing progress summaries directly to your office. If you have questions about whether your patient is a good candidate for in-home concierge PT, Anthony is available to discuss the case directly.
