In-home physical therapy after a stroke helps survivors relearn movement, improve safety, and regain independence in the exact environment where they live every day. With Anthony Daffner-Milos and A.D.M. Doctor of Physical Therapy, LLC, that care is delivered as a high-level, one-on-one concierge service tailored to complex neurologic needs like stroke.
Anthony Daffner-Milos delivers care in your home so that we can focus on function in your environment and not simulate it off-site. We will practice whatever is important to you and what is related to your goals. It may be getting in/out of your favorite chair, your bed, your bathroom, your kitchen, your lanai, your dock, your boat, your car; any task or location that is important to you or recommendations will be made to modify until you are ready for those tasks. Being treated at home reduces travel burden, fall risk during transportation, and sensory overload, which is especially important for people with fatigue, cognitive changes, or anxiety after a stroke.
| In-home physical therapy may be helpful when… | Clinic-based therapy may be a better fit when… |
|---|---|
| Travel is difficult or creates a major burden | Specialized equipment or a larger rehab team is needed |
| The main challenges involve mobility and safety at home | The person can travel safely and prefers a clinic setting |
| Caregivers need hands-on instruction in the home environment | Higher-intensity or more frequent medical services are required |
| The therapist needs to assess stairs, bathroom access, lighting, or fall hazards | The treatment plan calls for services not available in the home |
The right setting depends on medical stability, functional needs, available support, transportation, and the recommendations of the treating care team.
Home- or community-based rehabilitation may be appropriate after a functional and living-environment assessment, but some people need daily nursing, regular medical intervention, specialized equipment, or broader interdisciplinary care.
At home, the therapist can directly assess and modify real-world hazards like loose rugs, poor lighting, or optimize the set-up of your environment, then educate the client/patient and family on how to navigate them safely. This context-specific approach often improves carryover and reduces the risk of rehospitalization due to falls or complications. In-home care allows longer, uninterrupted one-on-one sessions and easier involvement of caregivers, which supports consistent practice and better adherence to home exercise prescriptions and recommendations to home modification.

For stroke survivors, Anthony focuses on restoring mobility (bed mobility, transfers, walking), balance, strength, and coordination, while also addressing dizziness, balance issues, and fall prevention in the home. Treatment is individualized, often integrating advanced techniques such as vestibular rehab and mechanical diagnosis and therapy when appropriate. The practice emphasizes aging in place and independent living, using PT to help stroke survivors remain safely at home, reduce reliance on assistive devices when possible, and improve confidence with everyday tasks like navigating stairs, entering the shower, or going outdoors.
The right therapy setting depends on the person’s medical needs, mobility, goals, transportation, and available support. In-home physical therapy may be especially helpful when practicing real-world tasks and assessing safety in the home are priorities.
